Retainer	$500 due and payable before the patient is seen in the office
Deposition	$750 minimum for two (2) hours or any part thereof in the doctor’s office $350 additional per hour or any part of an hour
Examination	$350 per hour
Report Preparation	$350 per hour
Review of Medical Records	$350 per hour
Psychological Testing - MMPI	$150
Telephone Consultation	$75 per 15 minutes
Court Testimony	$1800 per half day in Los Angeles County $3500 per whole day in Los Angeles County
Travel outside Los Angeles County to be compensated in addition to above plus travel expenses. Payment for court testimony is to be paid twenty-four (24) hours in advance of testimony. Payment for deposition is to be paid at time of deposition.

Medication (Psychopharmacology) and Treatment

Initial Consultation: 1 hour	$300
Subsequent visit (45 minutes)	$200
Subsequent visit (25) minutes)	$150

Sexual Identity

Initial Consultation: 1 hour	$300
Subsequent visit (45 minutes)	$200

Patients are expected to pay for the initial consultation at time of their initial visit and insurance will be billed as a courtesy.
Co-payments are payable at time service is provided on subsequent visits.
Patients are responsible for professional fees.
